About This Survey
COVD’s Quality of Life survey was developed in 1995 as a way to help establish the likelihood of a vision problem being present by analyzing the frequency with which symptoms occured. While these symptoms are not exclusive to vision problems, there is a high correlation between diagnosed vision problems and high scores on this survey.
The original 30 question survey was narrowed to the 19 questions you will find here. It has been found that a score of greater than or equal to 20 points indicates a high liklihood that a vision condition is present.
How to Use This Survey:
Take the survey (if you are taking the test for your child, ask for their input to each question, explaining each question, as needed). Click submit and view the results and further recommendations based on the results.
